The Clinical Care Coordinator of Private Duty Nursing ("Coordinator") assists the Location Director in managing the clinical integrity of! the in-home nursing program
Responsibilities:
Supervise assigned nursing personnel to include orientation and on-going evaluations.
Maintain employee files for assigned nursing personnel.
Participate in the screening, interview, and hiring process.
Request and maintain timely documentation for patient care to include physician orders, letters of medical necessity, prior authorization, insurance verification, etc.
Proactively manage risk for patients and personnel.
Monthly supervisory visits.
Acts as company and patient advocate in all aspects of care.
Create and maintain patient/staff schedules.
Obtain personnel certification and licensure documentation upon expiration.
Completes Performance Improvement activities per company policy. Assists in ensuring location is compliant with all state and federal guidelines.
Serve as liaison between personnel and patient/family for services provided.
Staff cases as requir! ed or assigned by Supervisor.
Maintain all appropriate cre! dentials, licensure, and certification as mandated by the position without restriction, sanction, or revocation.
Fosters a positive work environment at all levels within the organization and externally
Assesses the family dynamics, strengths and weaknesses, particularly in regard to the patient's medical condition and developmental status.
Provides teaching, counseling and support to families in an effort to facilitate adjustment to their child's medical problems and/or developmental status.
Evaluates and updates nursing care plan/treatment plan, establishing new goals according to patient progress.
Maintains awareness of and complies with company policies and procedures.
Acts in accordance with the Nurse Practice Act while complying with company policy and procedure.
Maintains professional, supportive and responsive interpersonal communication skills.
Demonstrates initiative, ability to problem solve and makes appropriate nursing judgments. !
Reflects current knowledge of trends, research and recent advances in the care of the medically complex child.
Uphold patient/family and personnel confidentiality as dictated by HIPAA.
Aware of pediatric emergencies and the interventions necessary.
Participate in after-hours or weekend 'on-call' process.
Other duties as assigned.
Qualifications:
Registered Nurse in state(s) serviced by location
Demonstrates adequate knowledge of home ventilators, apnea monitors, oxygen, trachs and trach care, suctioning, special feedings, G-tubes, N/G tubes, J-tubes and IVs.
Annual Basic Life Support Certification.
Two years experience in PICU, NICU or other related "high tech" environment .
Knowledge in Microsoft Office applications.
