The Nurse Case Manager provides clinically and evidenced based patient care coordination. Support the delivery of safe, effective, high quality and efficient patient care at Childrenâ! s Healthcare of Atlanta. Responsible for the coordination of assessment, interdisciplinary discharge planning and implementation of home health care and related services to assigned patients who are discharged from a CHOA facility.
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
Completes an initial screen of all patients on admission (not to exceed within 24 hours of admission) utilizing specific trigger criteria to identify needs related to care coordination and/or discharge planning.
Develops, initiates and implements a robust discharge care plan in collaboration with the clinical team for all applicable patients.
Cultivates and maintains effective interaction/communication with members of the medical staff, nursing staff, social workers, UR Team and peers to drive the care coordination process and to facilitate continuity of patient care.
Communicates with all members of the multi-disciplinary team to facilitate the care coordi! nation process for assigned workload.
Communicates wi! th home health agencies, third party payers and other community resources as needed to coordinate discharge needs.
Facilitates and provides on-going communication with patient/family and interdisciplinary staff to identify and resolve potential barriers to discharge.
Facilitates care conferences, interdisciplinary rounds, and catastrophic meetings.
Participates in focused system initiatives and facilitates clinical pathways at the patient level.
Collaborates with and is a resource for the Utilization Review Department staff as appropriate.
Performs outpatient and clinic care coordination and monitors patients care as they transition between outpatient and inpatient services where appropriate.
Refers cases identified as risk management issuers, peer review issues, or quality issues to the appropriate personnel.
Performs other responsibilities as required.
MINIMUM QUALIFICATIONS:
Bachelor of Sc! ience in Nursing or healthcare related field required. Masters degree preferred.
Registered Nurse with current Georgia license required or currently employed LPN Case Manager with equivalent experience and Georgia license required
Minimum of (5) years of broad clinical experience, predom
i
nantly in pediatric care. Care coordination, case management, discharge planning and utilization review experience preferred.
Excellent communication skills, both verbal and written.
Effective decision-making /problem-solving skills, demonstration of creativity in problem-solving, influential leadership skill.
Demonstrated effective critical thinking skills and ability to anticipate patient discharge needs.
Moderate to expert computer skills
Working knowledge of the financial aspects of third party payers and reimbursement.
Must be able to successfully pass the Basic Windows Skill Assessment at 80% o! r higher rating within 30 days of date of hire.
Registered Nur! se successfully obtains Certified Case Management (CCM) certification within 12 months of becoming eligible to be certified. Eligibility is determined by leadership review of national CCM standards and hours worked. Certification must be maintained on an ongoing, non-interrupted basis. CLPN Case Manager successfully obtains and maintains Certified Professional in Healthcare Management (CPHM) on an ongoing non-interrupted basis.
