Serve as the day-to-day lead within the Total Joint Replacement Center and supervise the development, implementation, and organization of the Destination Total Join! t Replacement Center program. The Joint Care Coordinator (JCC) will coordinate the efforts between surgeons, hospital administration and staff while tracking the progress of the program and identifying areas requiring attention. The JCC is responsible for all program management, report generation and review, process improvement, patient satisfaction, facilities review, budget (if required) and administrative activities within the Center. Develop, coordinate and provide staff education.

Job Responsibilities:
1. Lead regularly scheduled meetings with hospital administrators, the unit medical director, other physicians and key staff.
2. Manage the build out and oversee the day-to-day facility operations within the Center.
3. Develop standardized care plans with the surgeons including: pre-op, post-op and discharge order sets.
4. Ensure appropriate pre-op preparation for each patient prior to surgery.
5. Develop and oversee the creation of pr! e and post-op teaching classes including the coordination of t! eaching materials with the physicians' office.
6. Create measurement system for tracking surgical outcomes and patient satisfaction, identifying areas that require improvement.
7. Provide key statistics at Leadership Team meetings and to other stakeholders as necessary. Statistics may include: outcomes, progress against budget, patient satisfaction, complications, denied days, hospital-wide survey results, nurse call back results, etc.

8. Coordinate patient satisfaction events:
a. Call backs (when, who, how to report results)
b. Luncheons
9. Oversee the development and on-going editing of unit-specific forms and patient education materials including: Nurse Discharge Checklists, Coach Discharge Checklists, Patient Guidebooks, Daily Newsletters, Wall Hangings, and other standardized materials for the Destination Center
10. Develop a community education and seminar program inclusive of schedules, presenters, content and materials
11.! Create and oversee an ongoing quality improvement plan
12. Identify patient amenities and secure necessary resources to administer the services
13. Serve as a clinical resource person for staff and members of the health care team.
14. Develop and/or revises policies, procedures and protocols and educate staff accordingly.
15. Coordinate formal/informal educational offerings for staff and members of the community.
16. Participate in rounds to ensure compliance with standards of practice.
17. Other duties as assigned to meet the goals and objectives of DeKalb Medical.

Qualifications:
Minimum Education, Experience and Licensure Required:
1! . Clinical background - R.N., P.T., PA-C, CRNP, preferred
2. Minimu! m of 3- 5 years of experience, orthopedic experience preferred.
3. Proven working relationship with surgeons and demonstrated success in relationship building
4. Experience with patient / staff education. Strong presentation skills preferred.
5. Proven leadership experience, having managed resources either directly or within the implementation of a new initiative
6. Strong detail-orientation and follow-up capacity
7. Excellent organizational skills
8. Ability to keep team focused on goals / timelines
9. Customer service focused
