SUMMARY (BASIC PURPOSE OF THE JOB) Responsible for answering calls and providing appropriate information to assist callers in assessing need for health services. Gives appropriate clinical options and assists in assessing primary care, specialty care or community resources. Educates caller on self-care advice and preventative measures. Demonstrates an understanding of the life span from neonates to geriatrics in providing advice using established protocols. SUMMARY (ESSENTIAL JOB DUTIES)
- The Advice Nurse utilizes advanced knowledge, skills, and expertise of a specific patient population as demonstrated in the following job functions:
- Assesses patient/family in a timely, systematic and organized manner via telephone inquiries from GHS customers.
- Records health history of the caller and nursing assessment of the caller's needs and follows documentation standards.
- References appropriate triage guidelines on the screen for all calls based on assessment information, triage parameters, clinical judgment and facilitates appropriate disposition.
- Creates progress notes to document the assessment of caller's needs, information provided, and disposition for all calls.
- Schedules call backs within 24 hours for appropriate situations such as emergency referrals, pediatric home care follow up, 911 follow ups to determine compliance with recommendations.
- Refers inquiries to the appropriate resources, referring to PCP, Neighborhood Health Centers, Specialty clinics, as appropriate to assessments.
- Maintains documented records for medical, legal, and statistical purposes.
- Provides On call physician information regarding patient's plan of care.
- Provides education to patient & family and other health care providers
- Obtains via phone Critical Reporting Information to read back & verify and disseminate to the appropriate physicians according to hospital policy.
